Community Step-Down Services

Community-based Step-Down Services provide support for people experiencing mental health issues, in a home-like setting close to their community, friends and family.
Our Step-down provision is a short term, community-based supported living service, with personalised care and support for those who no longer require acute impatient level clinical intervention and treatment.
We aim to empower people take control of their mental health with prevention and recovery-focused care, allowing them to return to their home and community in a safe and supported way. This helps to facilitate early discharge from hospital, reduce the length of stay on acute mental health wards and improves flow within pathway.
Step-Down offers a holistic approach to care, with a multi-disciplinary team supporting different elements of the service user's recovery journey, including:

  • Monitoring of mental state
  • Activities of Daily Living skills development
  • Positive family and social support networks
  • Relapse prevention and risk reduction
  • Independent living
  • Least restrictive care and support
  • Flexible service which responds to their individual preferences and needs
  • Signposting to community groups, volunteer services and counselling services

Referrals are welcome from a range of sources including Adult Mental Health Services, Home Treatment and Acute Response Team, Acute Adult Mental Health Inpatient Units.
