We provide community-based services for adults with mental health conditions and intellectual disabilities, offering compassionate care as an alternative to long-term hospitalization. Our goal is to support rehabilitation within a community setting, promoting independence and quality of life.
With extensive experience in residential care, supported living, and home-based services, we tailor our programs to meet each individual's unique needs. Our recovery-focused approach builds on personal strengths, fostering meaningful progress and well-being.
Many of those we support have complex care needs, including co-existing mental health and learning disabilities. Our dedicated teams create a therapeutic environment that prioritises high-quality care by promoting independence, preserving dignity, respecting personal choice, and ensuring privacy.
We work closely with the local Community Mental Health Team, Integrated Care Board, hospitals, and the NHS to provide coordinated, person-cantered care.
Cognithan has exceptional, dedicated teams working to provide tailored care services for our service users.
We are committed to providing excellent social and community based care.

Our Board of Directors is committed to building strong relationships within our communities and with NHS partners. Representing the interests of service users, staff, and the wider community, the Board fosters a safe, supportive, and empowering environment for all.
The Board is responsible for setting our strategic direction, overseeing organizational performance, and ensuring effective governance and risk management. Key areas of responsibility include:
We continuously evaluate our performance against established quality indicators, striving for excellence across all aspects of social care provision.
We assess every potential service user on an individual basis to see if supported living is the right service for them. They have to be capable of keeping themselves safe under little supervision and have mastered all of the independent living skills that we've taught them.
Every supported living service user is assigned a support worker who checks in with them on a regular basis. They will make sure that the service user is achieving their agreed weekly goals. If the service user is having any trouble, then their support worker will help them get back on track.
We always discuss care plans with individual service users so that they can let us know exactly what they want and need from their support plan.
Each service user has access to advocacy services, should they need it. We have strong links with local advocacy groups and services.
Each service user is given their own bedroom, which we encourage them to personalise so that they feel as comfortable as possible when spending time there
All staff are fully qualified to fulfil their role in our services. Support Worker (NVQ II/ NVQ III), NVQ V; Registered Nurses (RNLD or RMN), Degree in Occupational health. In addition, we provide opportunities for relevant education relating to their job function.
This is based on the individual needs of each service user. Staff, with input from their care coordinator, assess whether the service user would be able to safely leave the premises unaccompanied. Family members are also consulted on this decision
