We can assist service users whose existing residential placements are not able to meet their needs and who may require increased intensive rehabilitation. Care can be tailored to individual’s needs, lifestyle and preferences.

Autism Assessment

Our Autism Assessment services are designed to help identify and understand the unique challenges individuals with autism face. We provide a comprehensive evaluation to help families and caregivers better support individuals with autism. The assessment helps determine the specific needs of each individual, enabling us to create personalized care plans that promote growth and well-being.


Autism and Learning Disability

We provide specialized services for individuals with both autism and learning disabilities. Our goal is to offer support that enables individuals to gain independence and participate fully in their community. Through tailored programs, we help enhance social, communication, and cognitive skills, while ensuring that individuals receive the support they need to thrive in their daily lives.


Home Care

What We Do

We offer a comprehensive range of services designed to help service users remain safely and comfortably in their own homes. Our approach is centered on the individual, prioritizing their preferences and support needs.
Essence2Care (E2C) provides compassionate, high-quality home care services tailored to fit your lifestyle and integrate seamlessly within your community.
Our services are customized to address specific needs related to medical conditions, personal preferences, and lifestyle. We support individuals who might otherwise require hospital care, enabling them to receive essential support at home.

Types of Services:

  • Personal Care: Assistance with bathing, showering, and dressing.
  • Domestic Assistance: Support with housework, including dusting, vacuuming, linen changes, laundry, and deep cleaning.
  • Light Gardening: Help with light gardening tasks such as weeding, pruning, and watering plants, making it easier for clients to enjoy their gardens.
  • Meals and Nutrition: Meal preparation and clean-up support.
  • Grocery Shopping: We can assist with errands, appointments, and events, helping you stay active and connected to your community.
  • Social Support: Companionship for activities like leisurely walks, engaging conversations, and reminiscing about favorite memories.
  • Respite Care: In-home respite care for primary caregivers, giving them the opportunity to recharge and take care of their own needs while ensuring their loved one is cared for.
  • Medication Reminders
  • Daily Living Support: Our drop-in services provide flexible assistance for individuals with disabilities, helping them live independently at home or transition to independent living arrangements.
  • Live-In and Overnight Support: E2C offers experienced care workers for live-in or 24-hour care. We provide flexible care options, from a few hours a week to overnight or round-the-clock support. Our services include:

    • Comprehensive care, from personal assistance to companionship and housekeeping, all in the comfort of your home

When a service user is referred to us, we will reach out to schedule a thorough assessment of their social care needs, lifestyle, medical conditions, and the support required to help them maintain their independence at home.
Following the assessment, we will create a personalized support and wellness plan tailored to the individual’s specific needs, challenges, and goals. Our priority is to provide a service that preserves their independence, supports their preferred routines, and helps them maintain their social and community connections.

Supported Living

Our Supported Living services are designed to help individuals live as independently as possible, while still receiving the support they need. We offer personalized care to meet the specific needs of each person, from assistance with daily activities to emotional support. Our goal is to empower individuals to live in their own homes or community settings, with tailored support that helps them grow and achieve their full potential.


Community Step-Down Services

Community-based Step-Down Services provide support for people experiencing mental health issues, in a home-like setting close to their community, friends and family.
Our Step-down provision is a short term, community-based supported living service, with personalised care and support for those who no longer require acute impatient level clinical intervention and treatment.
We aim to empower people take control of their mental health with prevention and recovery-focused care, allowing them to return to their home and community in a safe and supported way. This helps to facilitate early discharge from hospital, reduce the length of stay on acute mental health wards and improves flow within pathway.
Step-Down offers a holistic approach to care, with a multi-disciplinary team supporting different elements of the service user’s recovery journey, including:

  • Monitoring of mental state
  • Activities of Daily Living skills development
  • Positive family and social support networks
  • Relapse prevention and risk reduction
  • Independent living
  • Least restrictive care and support
  • Flexible service which responds to their individual preferences and needs
  • Signposting to community groups, volunteer services and counselling services

Referrals are welcome from a range of sources including Adult Mental Health Services, Home Treatment and Acute Response Team, Acute Adult Mental Health Inpatient Units.
